CHARTING THE FUTURE OF HEALTHY COMMUNITIES AND AGING 

 

Any one individual’s health status is largely the result of their actions, as well as the social and cultural context within which they live (their “community”). The mission of the Center for Community Health and Aging is to increase the capacity of individuals, organizations, and communities, to improve population health and aging across the life-course. Over its twenty-year history, the center has been involved in more than 150 projects, with funding of more than $80 million. Project topics include evidence-based health and wellness programs, training and development, data and technological innovations, program evaluation, community capacity building, economics and policy, community health assessment, among others. These proj­ects reflect the center’s innovations in research, educa­tion and practice that examine social, behavior­al, economic, environmental, and technological solutions linking academic and community settings. 

We've seen tremendous progress in the health and wellbeing of our participants who have gone through the Live in Control curriculum, It has been a joy and a blessing to see the "lightbulb" go on when our participants understand that they can do it. They can live healthy and happy while managing their diabetes. And we would tell them in class that they don't have to give up all the foods they love, and throughout the 4 sessions, they start to understand how that is. Then at that point, we see their whole countenance change once they get that. In short, we really want to continue to touch lives with the Live in Control curriculum. We've seen in work and change people!

Our patients have lowered their AIC from over 10 to less than 7! The education surrounding "how to manage your diabetes" as a patient has been invaluable.
