The Center for Community Health Development completed the CHI St. Luke’s Memorial-Health Community Needs Assessment in March 2019. This region includes the counties of Angelina, Houston, Jasper, Polk, Sabine, San Augustine, and Trinity.
2019 CHI St. Luke's Memorial-Health Assessment ReportIn 2001, the Center for Community Health Development initiated a plan to utilize a community health development approach (CHD) to improve health in the Brazos Valley. The center led the community through the CHD process of assessment, planning, and implementation to develop locally supported, sustainable health solutions called community resource centers. In 2017, the Episcopal Health Foundation contracted with the center to document the history of the community resource centers through a Case Study as well as create a Toolkit for replication and/or adaptation to develop similar resource centers.
